A program for pregnant women and parents of young children.
About MCH
Some families with young children need more help to get the services they need. If you need help, a staff in the program can tell you about the services available and how to get them. They will work with you to:
- make sure you get the services you need
- develop a service plan
- assist you with arranging appointments and transportation
The SF-MCH program assists families to access other supports and health services within the community or externally. This program provides home visits to families by nurses and specially trained home visitors. We provide referrals and access to other service supports and promote coordination of services for children and families with complex needs.
Objectives
- to promote the physical, emotional, mental and spiritual wellbeing of individuals within the family and families as a whole;
- to promote trusting and supportive relationships between parents and children, care providers and family, and among resources within the community;
- to increase community’s capacity to support families;
- and to empower families.
The Strengthening Families Maternal Child Health Program is a family focused home visiting program for pregnant women, fathers, and families of infants and young children aged 0-6 years of age.
